Initial Visit: We strive to make your initial visit to Innate as simple as possible. In an attempt to do so, you may download your new patient paperwork here. You are also welcome to pick up your paperwork at our office prior to your initial visit. If you chose to complete the forms on the day of your first visit, please arrive 10 to 15 minutes prior to your appointment time.

The new patient paperwork includes the following:

  • The Patient Intake Form and Review of Systems provide personalized information, current complaint, and health history. There are adult (ages 4 and over) and pediatric (ages 4 and under) forms available.
  • The HIPAA Notice of Privacy Practices outlines how your protected health information may be used and disclosed by Dr. Cassandra Baar,  the office staff, and other practitioners providing services to you, to pay your health care bills, to authenticate the doctors' treatment practices, and any other use required by law.
  • The Financial Policy outlines our payment policies used in the office.
  • The Informed Consent to Exam & Treat explains treatment risks and benefits. This is a 2-page document with multiple areas that require initials, check marks and signatures. Signing the Informed Consent to Exam & Treat gives Dr. Baar  permission to examine and treat you.
  • The Informed Consent to Communicate Electronically explains HIPAA privacy practices as it concerns electronic communication, specifically email; this may also apply to communication via Facebook and other social media platforms. 

Other forms may include neck or back questionnaires or motor vehicle accident information depending on the nature of the injury and complaint.

During your initial visit, the doctor will spend 30-60 minutes with you completing a detailed health history and a thorough examination. If x-rays are necessary, the doctor will refer you to an x-ray facility. If chiropractic care is appropriate, the doctor may choose to treat you during this initial visit. If another practitioner is necessary, she will refer you to the appropriate provider. She will make recommendations to aid in recovery and prevent recurrence. Dr. Baar will explain the treatment procedure, what takes place during the treatment, and why the treatment is being performed. Dr. Baar is always open to answering questions and addressing any concerns you might have at any time throughout your care.
